Mount Hope Centre for Long Term Care - London, ON

Part Time

Salary Range: $ $39.19 /hour

The RPN within Restorative Care provides restorative nursing care to support residents in maintaining and improving functional abilities, in accordance with the Fixing Long-Term Care Act, 2021, regulatory requirements, and best practices. The RPN will take a resident centered approach when working with the residents of Mount Hope. They will provide direction and guidance to the restorative care team to ensure the interventions are completed with the residents as per the care plan.

Key Responsibilities Include

  • Complete comprehensive assessments and develop, implement, and update individualized restorative care plans with appropriate documentation.
  • Continually evaluate restorative programs and communicate progress and changes effectively with the resident, Substitute Decision Maker (SDM), and interdisciplinary team members.
  • Collaborate closely with Physiotherapy and PSWs within Restorative Care to coordinate, implement, and evaluate restorative interventions, ensuring consistency with goals and resident Plans of Care.
  • Track and monitor progress, outcomes, and scheduling of the restorative caseload to ensure consistency and effectiveness of interventions.
  • Provide training, education, and ongoing coaching to staff to support restorative approaches in daily care.
  • Promote resident independence, dignity, and safety while preventing functional decline.
  • Ensure documentation aligns with the residents? Plan of Care, RAI-MDS, and legislative standards.
  • Demonstrate a committed, enthusiastic, and collaborative approach to restorative nursing practice.
  • Participate in quality improvement and restorative program evaluations.

It is understood that the hours of work reflect the current scheduling arrangements and are subject to change as determined by the Employer. You will have the opportunity to follow the collective agreement to pick up additional shifts in a home area RPN capacity.

Essential Qualifications

  • Current registration with the College of Nurses of Ontario as a Registered Practical Nurse
  • A minimum of 2 years' experience in a long-term care environment
  • Demonstrates knowledge of the Fixing Long Term Care Act, 2021 and applicable regulations
  • Demonstrates knowledge of restorative care and or nursing in rehabilitation.
  • Valid CPR certificate
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Demonstrated knowledge in assessment and care planning processes
  • Well-developed oral and written communication skills
  • Well organized and excellent time management skills along with attention to detail
  • Effective problem-solving skills
  • Proficiency in computer skills including experience with Point Click Care software
  • Proficiency and experience in pharmacology and medication administration
  • Recent experience with catheter injections, irrigations, sub q and IM injections, and G tube care maintenance.
  • Understanding of and recent relevant experience in use of asepsis
  • Previous experience in teaching and adult learning
  • A police record check completed within the last six (6) months from a Canadian Police Information Centre (CPIC), inclusive of vulnerable sector screening, will be required of the successful candidate

Immunization Requirements

  • Provide vaccination records or proof of immunity against measles, mumps, rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
  • Provide documentation of the Tuberculosis skin testing
