What You'll Do
In this full-time, cross-site role, you will:
Clinical Leadership & Resident Care
- Serve as a primary care provider, completing comprehensive health assessments for new and existing residents.
- Perform quarterly medication reviews, diagnostic investigations, and develop evidence-based treatment plans.
- Manage residents at high risk for falls, wounds, responsive behaviours, pain, and other clinical complexities.
- Provide skilled palliative and end-of-life care, ensuring dignity and comfort.
Collaboration & Consultation
- Work closely with the Medical Director, Director of Nursing, physicians, clinical managers, and interdisciplinary team.
- Consult on challenging clinical situations and support staff with best-practice approaches.
- Meet with families and substitute decision-makers to discuss care goals and treatment plans.
Education & Quality Improvement
- Act as a resource and educator to nursing staff on clinical programs, skin/wound care, and regulatory requirements.
- Participate in Resident Care Conferences, PAC, CQI, Accreditation processes, and risk management programs.
- Identify trends in care outcomes and support the team in implementing innovative strategies for resident well-being.
Documentation & Compliance
- Maintain accurate, timely documentation aligned with CNO standards and long-term care legislation.
- Ensure compliance with the Fixing Long-Term Care Act, 2021, and Ontario Regulation 246/22.
What We're Looking For
Education & Credentials
- Current registration with the College of Nurses of Ontario as an NP (Extended Class).
- Post-graduate education or certification in Gerontology or Palliative Care (assets).
Experience
- Minimum 1 year of experience in a long-term care setting.
- Background in adult education—including planning, delivering, and evaluating in-services.
Knowledge & Skills
- Strong understanding of geriatric, rehabilitation, and long-term care nursing.
- Knowledge of Ontario LTC standards and relevant provincial and federal legislation.
- Excellent communication, leadership, assessment, and organizational skills.
- Ability to collaborate effectively and support interdisciplinary teamwork.
Job Type: Full-time
Pay: $68.00-$70.00 per hour
