Role Description
This is a full-time, on-site Nurse Practitioner role based in Markham, ON. The Nurse Practitioner will provide high-quality patient care by conducting assessments, diagnosing conditions, creating treatment plans, and managing patient progress. Responsibilities include collaborating with a multidisciplinary team, utilizing clinical decision support tools, and documenting care in alignment with guidelines. Qualifications
Strong clinical skills, including patient assessment, diagnosis, and treatment planning
Experience with chronic disease management, medical guidelines, and referral coordination
Familiarity with clinical decision support platforms and electronic health record (EHR) systems
Excellent communication and interpersonal skills for collaboration with healthcare teams and patient engagement
Relevant licensing and certification as a Nurse Practitioner
