Role Description
This is a part time on-site role for a CDE Pharmacist based in Oshawa, ON. The CDE Pharmacist will provide patient-centered care, especially to individuals managing diabetes, including medication therapy management, counseling, and education on diabetes self-management. Responsibilities will include collaborating with healthcare teams to optimize patient outcomes, monitoring patient progress, conducting regular follow-ups, and supporting the development of individualized care plans. This is a clinical, non-dispensing role, however knowledge of Kroll is important.
Qualifications
- Expertise in Diabetes Education and Management; CDE (Certified Diabetes Educator) certification is required.
- Strong Pharmaceutical Knowledge, including medication therapy management for diabetes patients and chronic disease management.
- Excellent Communication Skills: Proven ability to provide patient counseling, education, and build strong patient-provider relationships.
- Collaboration and Teamwork: Ability to work effectively with healthcare teams to deliver quality patient care.
- Skills in Problem-Solving and Critical Thinking for managing personalized treatment plans and ensuring optimal outcomes.
- Proven ability to follow regulatory compliance and adhere to healthcare industry standards.
- Proficiency in using healthcare technology and systems to manage medical records and patient information effectively.
- Bachelor's degree in Pharmacy or Doctor of Pharmacy (PharmD) and current licensure to practice as a pharmacist in Ontario, Canada.
