The Village of Ridgeview Court located in Milton, Ontario has an exciting opportunity for an Assistant Director Nursing Care (ADNC) to join their Village providing enhanced clinical leadership to the Enhanced Supportive Neighbourhood.
Status: Permanent Full Time
Hiring Range: $67,980 - $76,450

Primary Focus Of The Role
This position is primarily accountable for:
Clinical excellence and practice standards
Complex case leadership
Dementia Care and mental health supports
System navigation and transitions
Clinical quality improvement
Ongoing specialized education and training
Partner and physician collaboration
Key Accountabilities
Clinical Leadership & Practice Excellence
Provide advanced clinical leadership in dementia care, mental health, and complex dementia care
Lead complex assessments, stabilization plans, and care conferences.
Guide non-pharmacological and least-restraint approaches.
Support medication optimization, pain management, and risk mitigation.
Serve as a clinical resource and mentor to nursing and care teams.
Interdisciplinary & Partner Collaboration
Lead collaboration with physicians, geriatricians, psychiatry, BSO, and community partners.
Coordinate complex case reviews and clinical consultations.
Ensure timely access to specialized supports.
Strengthen integration between BSU/ESN, other LTC homes, and acute care partners.
System Flow & Transitions (Clinical Focus)
Provide clinical oversight for admissions
Review/Approve all resident applications for BSU/ESN Lead clinical readiness assessments for LTC placement.
Oversee clinical components of LTC applications.
Support transition planning and handover to receiving homes.
Advocate for residents during placement discussions.
Quality, Risk & Clinical Governance
Monitor clinical outcomes (stabilization, personal expressions / responsive behaviours, incidents, readmissions).
Lead clinical audits, reviews, and improvement initiatives.
Support compliance with professional and regulatory standards.
Contribute to evaluation, research, and reporting.
Team Development, Education & Clinical Capacity Building
Build and sustain clinical capability in dementia care, mental health, and personal expressions.
Coordinate and support ongoing specialized training aligned with ESN resident profiles and emerging best practices.
Partner with internal education teams and external specialists to strengthen clinical learning.
Lead education, coaching, and reflective practice.
Support team member resilience and trauma-informed care.
Promote consistent, relational approaches to care.
Family & Care Partner Partnership (Clinical Lens)
Support complex clinical conversations with families.
Participate in care planning and transition discussions.
Provide education regarding behaviours, progression, and supports.
Strengthen trust and shared decision-making.
Strategic Program Development & Advocacy
Contribute clinical expertise to sustainability and expansion planning.
Support funding, system planning, and policy discussions.
Promote the ESN model internally and externally.
Focus on destigmatizing of dementia and personal expressions
Collaborative Leadership Model
The ADNC ESN works in a collaborative partnership with NC (Neighbourhood Coordinator).
Required
Qualifications & Experience
Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario.
Minimum five (5) years of clinical experience in Long Term Care, dementia care, mental health, or behavioural supports.
Minimum three (3) years in clinical leadership, advanced practice, or supervisory roles.
Demonstrated expertise in personal expressions (responsive behaviours) and stabilization models.
Strong knowledge of relational, person-centred, and trauma-informed care.
Preferred
Experience in transitional care, BSTU/ESN, or specialized neighbourhood models.
Clinical leadership experience in Long Term Care
Advanced training in gerontology, dementia, mental health, or behavioural supports.
Experience in clinical education, Team Member development, or practice coaching.
Quality improvement and system navigation experience.
Experience working with hospitals and placement agencies.
